An orthopedic surgeon and part owner of a successful surgery center in Florida was making $1,200,000 annually. His clinic provided taxable group disability benefits capped at $25,000 per month. He also had an additional layer of individual coverage from a domestic U.S. carrier providing him with another $25,000 per month.
